This user permit application is for parks and facilities managed by the Durango Parks and Recreation Department. Permits must be received a minimum of 10 days prior to the activity.

A $100 non-refundable application fee is required for Rotary Park and a $50 non-refundable fee is required for all other City parks in addition to a facility damage deposit. If there is no damage to the facility, the deposit will be returned to the first person and address listed below no sooner than 7 days following the date of the event.

The number of participants is used to determine your damage deposit. Please be as accurate as possible.

If amplifiers are to be used, a "Noise Variance" — which is not part of this permit — must be obtained from the Police Department.

If the event is for monetary gain, the user is responsible to pay additional fees of $100 or 15% of net revenue (whichever is greater) upon completion of event. Receipt must accompany fee payable to "City of Durango" c/o Parks and Recreation. 949 E. 2nd Ave. Durango, CO 81301.

Request for Alcohol at City Facility - City Ordinance #3-10 prohibits alcoholic beverages in City parks, with the exception of the Lion's Den.

NOTE: If fermented malt beverages are to be sold at this function, the applicant must apply to the City Clerk for a special events permit. Insurance will also be required. It is illegal to sell fermented malt beverages without said permit. Application for the permit must be filed with the City Clerk at least forty-five (45) days prior to the scheduled event.

1. The provisions of City Ordinance section 18-34 shall apply.
2. If any food or merchandise is to be sold, applicant must attach to this permit, thirty (30) days prior to proposed activity, a letter of intent and receive approval as specified in City Ordinances section 18-34.
3. There shall be no blocking of streets without acquiring City authorization.
4. No stages or platforms are permitted unless approved by City Building and Fire Codes.
5. City Ordinance sections 16 – 4 prohibits loud noises. A noise ordinance variance must be obtained from the Police Department for use of amplifiers.
6. Persons reserving this facility that does NOT have restrooms provided, are responsible for providing portable restrooms at their own expense.
7. The provisions of City Ordinance section 4-28 shall apply. It shall be unlawful for any person to ride, lead or drive horses upon this facility.

I accept responsibility for the above conditions and agree to make restitution for any damage to the park or City property. I hereby exempt and release the City of Durango, its officers, employees, agents and servants from any and all loss, liability, claims, demands, actions, and causes of actions whatsoever arising out of any loss, damage or injury that may occur as a result of the referenced facility use requested herein.
